Acute epididymo-orchitis
Usually due to Gram negative enteric bacteria (e.g. E.coli, Klebsiella, Proteus etc) in men>35 years with lower risk of STI.
If under 35 years or STI risk, refer to Sexual Health Clinic.
Send MSU (+ First Void Urine for Chlamydia/Gonorrhoea PCR if STI risk) prior to empirical antibiotics.
Consider ultrasound or surgical assessment if there is little improvement on antibiotic therapy
