First Line

Therapy

Vancomycin IV + gentamicin IV

Notes:

1. See individual treatment protocols on ward - ensure vancomcyin added if not part of protocol- contact specialist centre for further advice

2. If neutropenic, follow neutropenic sepsis guidance BUT add vancomycin IV

3. If patient receiving parenteral nutrition - consider antifungal treatment - discuss with microbiology
