Respiratory Tract Infections

First Line

Amoxicillin 500mg PO 8 hourly 

 

Escalation or severe infection / sepsis:

Co-amoxiclav 1.2g IV 8 hourly + Erythromycin (if atypical pneumonia is suspected/likely or confirmed) 500mg PO 6 hourly  

Penicillin Allergy

Erythromycin 500mg PO 6 hourly

 

Escalation or severe infection / sepsis:

Ceftriaxone 2g IV daily + Erythromycin (if atypical pneumonia is suspected/likely or confirmed) 500mg PO 6 hourly

Notes

Erythromycin may cause GI upset in breastfed infants

During the influenza season ensure viral throat swabs/gargles are sent and consider using antivirals if clinically indicated

 

Treatment duration: 5 days 

 

Oral switch guidance

Review culture results first. If no positive microbiology results to guide:

Preferred regime - Co-amoxiclav 625mg PO 8 hourly +/-  Erythromycin 500mg PO 6 hourly

Alternative - Cefuroxime 500mg PO 12 hourly +/- Erythromycin 500mg PO 6 hourly
